Who is the data controller, that is the organisation in control of processing your information under the Data Protection Act 1998?

What information is collected?

We collect:

• Information about our service users

• Information in case notes that we develop on our service users in providing our service

• Contact details of their parents, guardians or carers

• Information on applicants for posts, including references

• Employee information – contact details, bank account number, payroll information, supervision and appraisal notes.

• Information about our trustees

How is information on our service users, their parents, guardians and carers collected?

• From your initial referral form

• From the child’s assessment

• From ongoing data collection related to the child’s individual programme targets

• From contact details for ongoing communication with you

How is information on employees, applicants for posts and trustees collected?

• From applications for posts, including references

• From employee information provided by employees and from providers of references

• From personal information provided by trustees

Why is collected?

It is collected to enable us to administer BSAP and to provide a service to our users

How will it be used?

• Information on applicants for posts, employees and trustees will be used to enable us to administer BSAP

• Information on service users, their parents, guardians or carers will only be used to enable us to provide a service to our users.

Who will it be shared with?

This information will not be shared with anyone except staff working with service users and the management team

What will be the effect of this on the individuals concerned?

Any information we collect is for the direct benefit of our service users and their families: e.g tracking and monitoring the child’s progress.

Do I have a right to see the information BSAP holds about me, as a parent, guardian or carer of a service user, and the child concerned, or about me as an employee, applicant for a post or trustee?

The following information will be required before access is granted:

• The full name and contact details of the person making the request

• Your relationship with BSAP (applicant for a post, former or current member of staff, trustee, or parent, guardian or carer of a service user, including the name of the service user

• Any other relevant information, e.g. timescales involved

We may also require proof of identity before access is granted. The following forms of ID may be required:

• Copy of the personal information page of a passport or copy of driver’s licence for an applicant for a post, former or current member of staff, trustee, or the service user’s parent, guardian or carer

We may make a charge of £10 on each occasion access is requested

Queries about handling personal information will be dealt with swiftly and politely.

We will aim to comply with requests for access to personal information as soon as possible, but will ensure it is provided within the 40 days required by the Data Protection Act from receiving the written request.
